About The Position
EasyHealth is growing its Nurse Practitioner network in Flagstaff, Arizona, and is seeking experienced, compassionate NPs to provide in-home preventive care. This role offers autonomy, meaningful patient interaction, and flexibility outside the traditional clinic setting. NPs will conduct comprehensive preventive health assessments during scheduled in-home visits to identify care needs, close gaps, and connect members to ongoing care. This is not a traditional primary care practice with a packed clinic schedule, allowing for focused, high-quality assessments.
Requirements
- Hold an active Arizona Nurse Practitioner license.
- Are nationally board certified.
- Must be certified Pediatric Nurse Practitioner.
- Must be registered Medicaid.
- Comfortable conducting comprehensive Medicaid health assessments.
- Comfortable identifying care gaps.
- Strong assessment and clinical decision-making skills.
- Comfortable traveling locally for scheduled in-home visits.
- Enjoy working independently while being part of a supportive clinical organization.
- Bring warmth, professionalism, and excellent communication skills into every member's home.
