About The Position
EasyHealth is growing its Nurse Practitioner network in Flagstaff, Arizona, and Washington, seeking experienced and compassionate NPs to provide in-home preventive care. This role offers autonomy, meaningful patient interaction, and flexibility outside the traditional clinic setting, focusing on conducting comprehensive preventive health assessments during scheduled in-home visits to identify care needs, close gaps, and connect members to ongoing care. It is not a traditional primary care practice or a packed clinic schedule, allowing NPs to focus on individual members and deliver high-quality assessments.
Requirements
- Active Arizona Nurse Practitioner license
- Nationally board certified
- Certified Pediatric Nurse Practitioner
- Registered Medicaid
- Comfortable conducting comprehensive Medicaid health assessments
- Comfortable identifying care gaps
- Strong assessment and clinical decision-making skills
- Comfortable traveling locally for scheduled in-home visits
- Enjoy working independently
- Bring warmth, professionalism, and excellent communication skills into every member's home
Responsibilities
