About The Position

Signify Health, part of CVS Health®, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships. The goal is to provide flexible, comprehensive, and personalized health evaluations directly to people's homes, improving health outcomes and connecting them to the right care for their unique needs. The evaluations typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and potentially diagnostic tests if ordered by the health plan. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can be diverse and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to perform their duties comfortably and safely in these environments.

Requirements

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Able to cover the following primary locations: Livingston, Rochester and surrounding area
• Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.

• Conduct In-Home Health Evaluations.

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
• Be flexible to travel locally and within licensed states, as needed.
