In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Saginaw, MI
About The Position
Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will provide flexible, comprehensive, and personalized health evaluations directly to individuals. The visits, lasting approximately 45 minutes, allow for one-on-one connection and include a medication and medical history review, a physical evaluation, and potentially diagnostic tests if ordered by the health plan. Clinicians will use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment plans. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, or other airborne allergens.

Responsibilities
- Conduct In-Home Health Evaluations.
- Engage with people in their homes to bridge gaps in care.
- Build trusted relationships to improve health outcomes.
- Provide flexible, comprehensive, and personalized health evaluations.
- Conduct medication and medical history reviews.
- Perform physical evaluations.
- Administer diagnostic tests as ordered by the health plan (e.g., diabetic eye exams, spirometry).
- Use an iPad and an intuitive clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
