In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) -Portland, WA
About The Position
Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to help bridge gaps in care and build trusted relationships to improve health outcomes. The position focuses on providing flexible, comprehensive, and personalized health evaluations directly to individuals. The evaluations typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and potentially additional diagnostic tests ordered by the health plan. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, or other airborne allergens.
Requirements
- Active, unrestricted license(s) in coverage area(s) PORTLAND
- Board certification (required)
- Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.
- Must be able to travel locally and within licensed states.

Responsibilities
- Conduct In-Home Health Evaluations.
- Engage with people in their homes to bridge gaps in care.
- Build trusted relationships to improve health outcomes.
- Provide flexible, comprehensive, and personalized health evaluations.
- Conduct medication and medical history reviews.
- Perform physical evaluations.
- Administer additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
- Utilize an iPad and clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
