PRN Nurse Practitioner or Physician Assistant
About The Position
The Community Health Center of Central Wyoming (CHCCW) is seeking a compassionate, skilled, and community-minded Nurse Practitioner (NP) or Physician Assistant (PA) to provide PRN coverage at our Dubois Clinic. Requirements

Nice To Haves
- Experience in family medicine, primary care, rural healthcare, or community health is preferred.
Responsibilities
- Provide comprehensive primary care services to patients across the lifespan
- Conduct patient histories, physical examinations, assessments, and evaluations
- Diagnose and treat acute and chronic health conditions
- Develop individualized treatment plans based on patient needs
- Order and interpret appropriate laboratory, diagnostic, and imaging studies
- Prescribe medications and treatments within the scope of practice
- Provide preventive care, health education, and counseling
- Monitor and manage chronic medical conditions
- Identify conditions requiring additional evaluation, specialty care, or a higher level of treatment and make appropriate referrals
- Collaborate with physicians, nurses, medical assistants, behavioral health professionals, and other members of the healthcare team
- Maintain accurate and timely medical records and clinical documentation
- Provide care in accordance with CHCCW policies, clinical guidelines, quality standards, and applicable laws and regulations
- Participate in quality improvement and patient safety activities as appropriate
- Maintain patient confidentiality and comply with HIPAA requirements
- Provide compassionate, culturally responsive care to a diverse patient population
