About The Position
Elaya Health is seeking a compassionate, highly skilled Nurse Practitioner or Physician Assistant to provide on-site endocrinology care to residents in skilled nursing and long-term care facilities across New York, New Jersey, and Florida. The provider will help improve outcomes for patients with diabetes and other endocrine disorders through proactive management, collaboration with facility staff, and clear communication with nursing and administration. The role includes clinical and administrative support, with training provided for facility electronic medical records.
Requirements
- Licensed Nurse Practitioner or Physician Assistant in New York or New Jersey, or willingness to obtain licensure.
- Strong communication skills and a patient-centered approach.
- Ability to work independently and collaboratively.
Nice To Haves
- Prior experience in skilled nursing, internal medicine, or endocrinology.
Responsibilities
- Perform on-site patient evaluations and follow-ups.
- Manage diabetes, thyroid disorders, and other common endocrine conditions.
- Collaborate with directors of nursing, facility leadership, and primary care teams.
- Document care in facility electronic medical records.
- Support initiatives to reduce unnecessary hospitalizations and improve A1C and finger-stick outcomes.
