Bronx, NY - PA or NP for Endocrinology Practice
About The Position
Elaya Health is seeking a compassionate, highly skilled Nurse Practitioner or Physician Assistant to join its growing endocrinology group, which provides care in skilled nursing and long-term care facilities. The practice serves residents in facilities across New York, New Jersey, and Florida. Providers help improve outcomes for patients with diabetes and other endocrine disorders through proactive management, collaboration with facility staff, and clear communication with nursing and administration. The role includes on-site patient evaluations and follow-ups, management of common endocrine conditions, and work with facility leadership and primary care teams. Facility EMR training is provided. Requirements
- Be licensed as a Nurse Practitioner or Physician Assistant in New York or New Jersey, or be willing to obtain licensure.
- Have strong communication skills and a patient-centered approach.
- Be able to work independently and collaboratively.
Nice To Haves
- Prior experience in skilled nursing, internal medicine, or endocrinology.
Responsibilities
- Perform on-site patient evaluations and follow-ups.
- Manage diabetes, thyroid disorders, and other common endocrine conditions.
- Collaborate with Directors of Nursing, facility leadership, and primary teams.
- Document care in facility EMRs; training is provided.
- Support initiatives to reduce unnecessary hospitalizations and improve A1C and finger-stick outcomes.
