About The Position

Join a collaborative and well-established Family Medicine team at Hartford HealthCare as a Nurse Practitioner or Physician Assistant in Primary Care. This supportive practice includes 4 physicians and 1 APP, an embedded behavioral health provider, and a clinical pharmacist, all working together to deliver comprehensive, patient-centered care. Hartford HealthCare supports Nurse Practitioners and Physician Assistants in practice with dedicated support and voice through the Office of Advanced Practice. Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients. In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists. Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

Requirements

  • Unrestricted State of Connecticut Medical License (or eligible)
  • APRN (Family Nurse Practitioner) or PA
  • 1+ years’ experience in a primary care or relevant outpatient setting

Responsibilities

  • Perform routine physical examinations.
  • Formulate medical, educational, and psychosocial plans to treat patients.
  • Develop plans for further evaluation of health needs in consultation with physicians and care team members.
  • Refer patients to other health team members or specialists as needed.
  • Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols.
  • Identify problems outside of their scope of practice and refer appropriately to a physician.
