About The Position

Horizon Health in Wessington Springs, South Dakota, is hiring two full-time Nurse Practitioners or Physician Assistants to provide direct patient care. The role coordinates primary care for patients and emphasizes high-quality, patient-centered care in a collaborative, team-oriented environment. Emergency room and hospital coverage are required, with weekday and weekend rotation. A base salary is provided, with additional compensation for ER call. Successful candidates must be licensed in South Dakota or able to obtain state licensure. Certification from an accredited organization is required before employment. ACLS, ATLS, and PALS certifications are required or must be obtained upon hire. Requirements

Must be licensed in South Dakota or able to successfully obtain South Dakota licensure.
Certification from an accredited organization is required before employment.
ACLS, ATLS, and PALS certifications are required or must be obtained upon hire.

Responsibilities

Provide primary health care according to established evidence-based clinical practice guidelines.
Lead care teams in delivering high-quality healthcare consistent with the Horizon Quality Improvement Plan and established goals and benchmarks.
Manage long-term care for acutely and chronically ill patients, including care coordination and development of care plans and self-management plans.
Provide emergency room and hospital coverage, including weekday and weekend rotation.
