Cleveland, OH - PA or NP for Endocrinology Practice
About The Position
Elaya Health is seeking an experienced, compassionate Nurse Practitioner or Physician Assistant to provide on-site endocrinology care to residents of skilled nursing and long-term care facilities throughout the Cleveland area. Providers manage diabetes, thyroid disease, and other common endocrine conditions, working closely with facility nursing teams, primary care providers, and administrators. The role offers clinical autonomy, flexible scheduling, administrative and clinical support, training in the practice’s protocols and workflows, and opportunities for advancement as the practice expands. Providers document care in facility electronic medical record systems and receive training and administrative support. Clinical leadership is provided by Medical Director Dr. Don Zwickler, an endocrinologist.
Requirements
- Licensed Nurse Practitioner or Physician Assistant in Ohio, or willingness to obtain Ohio licensure.
- Strong clinical judgment and communication skills.
- Comfortable working independently while collaborating with a larger clinical team.
- Patient-centered and committed to high-quality care for the long-term care population.
Nice To Haves
- Experience in endocrinology, internal medicine, geriatrics, or skilled nursing is preferred.
- Endocrinology experience is a plus but is not required for the right candidate.
Responsibilities
- Perform on-site patient evaluations and follow-up visits.
- Manage diabetes, thyroid disorders, and other common endocrine conditions.
- Review blood glucose trends, laboratory results, and medication regimens.
- Optimize diabetes management and help reduce unnecessary finger sticks and medication burden.
- Collaborate with nursing staff, directors of nursing, facility leadership, and primary care teams.
- Provide clear clinical recommendations and ongoing follow-up.
- Document in facility electronic medical records.
- Help reduce avoidable hospitalizations through proactive specialty care.
