About The Position

Humana's Primary Care Organization is seeking a full-time Float Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at our clinic in Orlando, FL. As a Nurse Practitioner (NP) or a Physician Assistant (PA) at CenterWell Senior Primary Care, you will be part of a collaborative, diverse team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. We're not just improving healthcare, we're improving lives. That includes yours. You will report to the Associate Medical Director and float as needed throughout our Orlando, FL market.

Requirements

• Master's degree in nursing with board certification, or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state of Florida.
• At least one year of experience in value-based care, primary care, or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with your care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at your center.
• Have experience with providing value-based care.
• Work on-site.
