About The Position

Emplify Health by Gundersen is seeking an experienced Nurse Practitioner or Physician Assistant to join its outpatient Family Medicine team at the La Crescent campus. This 0.8 FTE position provides comprehensive, patient-centered care in a collaborative clinical environment. Clinic hours are Monday through Friday, 7:30 a.m. to 4:30 p.m. The La Crescent campus offers primary care services and connects clinicians with specialty and support services across Emplify Health by Gundersen, including family medicine, internal medicine, physical therapy, integrated behavioral health, laboratory, and imaging services. Advanced practice clinicians work under the direct supervision of a physician. Clinicians may be asked to provide outreach services, and teaching students, residents, fellows, or clinical staff is an expectation of all clinicians. Emplify Health by Gundersen is a physician-led, not-for-profit healthcare system serving communities across western Wisconsin, southeastern Minnesota, and northeastern Iowa. It is part of Emplify Health, a patient-centered network that includes Bellin Health and Gundersen Health System.

Requirements

Be an experienced Nurse Practitioner or Physician Assistant.
Have completed an accredited program.
Be eligible to obtain a license to practice in the multi-state region, with no restrictions or limitations.

Responsibilities

Provide comprehensive, patient-centered Family Medicine care in an outpatient setting.
Work collaboratively with the clinic team and the health system's specialty and support services.
Work under the direct supervision of a physician.
Provide outreach services as needed as part of the role.
Teach students, residents, fellows, or clinical staff.
