About The Position

Emplify Health by Gundersen is seeking an experienced Nurse Practitioner or Physician Assistant to join its Family Medicine team at the La Crescent campus. This 0.8 FTE role provides comprehensive, patient-centered care in an outpatient-only setting. Clinic hours are Monday through Friday, 7:30 a.m. to 4:30 p.m. The La Crescent campus offers comprehensive primary care and access to the organization’s specialty and support services. Providers work in an integrated, collaborative environment alongside family medicine, internal medicine, physical therapy, integrated behavioral health, laboratory, and imaging services. Advanced practice clinicians work under the direct supervision of a physician. Clinicians may be asked to provide outreach services. Teaching students, residents, fellows, or clinical staff is an expectation of all clinicians. Emplify Health by Gundersen is a physician-led, not-for-profit healthcare system serving communities across western Wisconsin, southeastern Minnesota, and northeastern Iowa. It is part of Emplify Health, a patient-centered network formed by Bellin Health and Gundersen Health System.

Requirements

Experienced Nurse Practitioner or Physician Assistant.
Must have completed an accredited program.
Must be eligible to obtain a license to practice in the multi-state region, with no restrictions or limitations.

Responsibilities

Provide comprehensive, patient-centered Family Medicine care in an outpatient-only setting.
Work collaboratively with the La Crescent campus care team and access available specialty and support services.
Work under the direct supervision of a physician.
Provide outreach services as needed as part of the role.
Teach students, residents, fellows, or clinical staff.
