About The Position

Neighborhood Health is seeking Nurse Practitioners - FNP for its network of community health centers in Nashville, Gallatin, and Lebanon, Tennessee. These centers provide comprehensive primary care, integrated behavioral health, and dental services to a diverse patient population, including immigrants, residents of public housing, individuals experiencing homelessness, and people living with addiction. As a federally qualified health center, Neighborhood Health provides care regardless of health insurance or ability to pay. The organization is committed to improving its Patient Centered Medical Home model to ensure high-quality, timely, and appropriate care. The Nurse Practitioner will be responsible for providing healthcare services within the health centers, adhering to protocols established with the Chief Clinical Officer. This may include ambulatory services, night call care, and skilled nursing facility care. The role involves participation in quality assurance, member satisfaction initiatives, efficient use of healthcare resources, and team meetings. The Nurse Practitioner will support the UNHS mission to ensure access to healthcare for the underserved, engage patients through a patient-centered health home model, deliver high-quality care, and help control healthcare costs. The position requires working within a Patient Centered Medical Home model, fostering collaboration between patients, physicians, and health teams with a focus on care coordination and integrated treatment. A commitment to providing accessible, continuous, comprehensive, coordinated, compassionate, and culturally effective services is essential, with a focus on eliminating barriers to care and prioritizing patient needs and convenience. The Nurse Practitioner will also support integrated healthcare by collaborating with behavioral health and dental health providers as needed and will be responsible for achieving meaningful use of electronic health records.

Requirements

  • R.N. and MSN education in primary care
  • Licensed/certified in the State of Tennessee as appropriate.
  • Maintain basic life support certifications.
  • Maintains current continuing nursing/medical education and requirements for privileging.
  • 2 to 3 years experience as a Nurse Practitioner.
  • Interpersonal skills to manage and develop the staff within the most immediate work group, and to participate on provider team.
  • Analytical ability to review internal processes needing improvement and make suggestions.
  • Verbal ability to communicate thoughts and actions to individual patients and to staff.
  • Able to provide the same level of care to patients and the same level of cooperation to staff regardless of age, race, color, creed, religion, sex, sexual preference, gender identification, national origin, health status, income level, or payment plan.

Responsibilities

  • Provision of health care services to patients who receive their care at UNHS health centers pursuant to protocols established with the Chief Clinical Officer.
  • Participation in quality assurance activity, member satisfaction, appropriate utilization of health care resources, and team meetings.
  • Support the UNHS mission to assure access of the underserved to healthcare, to engage patients in a meaningful healthcare experience through the patient-centered health home model, to provide high quality healthcare and to participate in controlling the cost of such care.

  • Facilitate partnerships between patients, physicians and health teams with focus on care coordination and integration of treatment internally and externally.
  • Work diligently to assure that services are accessible, continuous, comprehensive, coordinated, compassionate, and culturally effective.
  • Be committed to eliminating barriers to care that is centered on the needs and convenience of patients above all other factors.
  • Support integrated healthcare involving behavioral health providers and dental health providers in care as appropriate.
  • Achieve meaningful use of electronic health records each year as the standards are updated and expanded by the federal government.
