The NP - Innovating Market Processes and Clinical Transformation (IMPACT) will play an essential role in the setup and operational establishment of new markets, as well as the stabilization of existing markets. This involves traveling to new regions to support market launches, train local teams, and ensure the delivery of high-quality care from day one. This dynamic role requires clinical expertise, flexibility, and the ability to adapt to different patient populations, community resources, and healthcare systems. The NP Impact provides high-quality, patient-centered care to patients in their home environment, focusing on delivering direct clinical care, including health assessments, treatment interventions, and care management to individuals with complex healthcare needs.

Requirements

  • Current Nurse Practitioner License. Must be able to obtain and maintain a Nurse Practitioner License within all service states within 6 months of hire.
  • Must have an active DEA license or be eligible to obtain DEA license
  • Active CPR or BLS Certification
  • Must maintain a valid driver’s license and good driving record
  • Outstanding EHR skills

Responsibilities

  • Serve as a key member of the IMPACT team in the initial setup and rollout of new markets, traveling to identified regions to launch and establish local care teams.
  • Collaborate with other IMPACT team members and local leadership to ensure that market setup is smooth, operational workflows are established, and new market teams are trained and supported.
  • Support the establishment of clinical best practices, protocols, and workflows in new markets to ensure the delivery of high-quality care that aligns with company standards.
  • Provide on-site training and mentorship to local clinicians, ensuring that they are fully prepared to deliver patient care in the home setting according to company care model principles.
  • Assist in the development and implementation of the care delivery infrastructure in new markets, including establishing relationships with local healthcare providers and community resources.
  • Assess existing market needs and implement strategies to address gaps in care and at risk team members, standardize operations, and enhance overall market performance.
  • Deliver direct patient care, including comprehensive health assessments, physical exams, diagnostic testing, and therapeutic interventions in the home.
  • Develop and implement personalized care plans based on patient needs, preferences, and clinical assessments, collaborating with the IMPACT team.
  • Monitor and evaluate patient progress, adjusting care plans as necessary to address any changes in health status or condition.
  • Administer treatments such as medication management, injections, wound care, and other clinical procedures, following all safety and infection control protocols.
  • Provide patient and family education regarding disease management, care plans, treatment options, and lifestyle adjustments to enhance health outcomes.
