Nurse Case Manager - Charlotte, NC
About The Position
Paradigm is seeking a Field Nurse Case Manager to support injured workers in Charlotte and surrounding communities. This is a hybrid position requiring regular local travel (80% of the time) for in-person patient appointments. The role involves the medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation. The Case Manager will collaborate with insurance carriers, medical providers, attorneys, employers, and employees, monitoring injured worker progress and reporting results. Candidates must be willing to learn and manage catastrophic cases.
Requirements
- Current, unencumbered compact Registered Nurse (RN) license in NC.
- Registered as a Rehabilitation Professional with the North Carolina Industrial Commission required within 60 days of hire.
- Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal).
- Ability to operate autonomously with minimal oversight.
- Skilled at patient education.
- Valid Driver’s License, and good driving history.
- Must be located in the Raleigh area.
- Must be willing to travel 80% of the time.
- Must be licensed in NC.
Nice To Haves
- Experience in case management of workers comp cases.
- Recommended certifications (CCM, CRRN, CDMS, CRC) preferred. REQUIRED within two years of the date of hire.
Responsibilities
- Medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals.
- Work with insurance carriers, medical care providers, attorneys, employers, and employees.
- Closely monitor the progress of the injured worker.
- Report results back to the employer and insurance carriers.
- Willingness to learn and manage catastrophic cases.
