About The Position
This full-time position with the Dorchester County Health Department provides resource navigation and connects community members to health services through the Mobile Integrated Health program. The Nurse Case Manager facilitates community-based follow-up for the program, participates in home visits to provide health education, conducts community outreach, serves as a community liaison, and assists with relevant community health planning committees. A valid driver's license is required for duties involving motor vehicle operation.
Requirements
- Bachelor’s degree in nursing from an accredited college or university; stated substitutions include a related-field bachelor’s degree plus one year of Registered Nurse experience, two years of Registered Nurse experience, or qualifying U.S. Armed Forces military service experience on a year-for-year basis.
- Current Registered Nurse license from the Maryland State Board of Nursing.
- Valid Maryland motor vehicle operator’s license if assigned duties requiring operation of a motor vehicle.
- Must provide the facility with a telephone number at which they can be reached.
