Reporting to the manager of Chronic Disease Management, the responsibilities include: providing inpatient and outpatient education, clinical and self-management support for patients and families living with cardiovascular disease; working in collaboration with the inter-professional team in acute care and in an outpatient setting; individual telephone follow-up post discharge; staff education; collaboration with other SHA departments.
