About The Position

Reporting to the manager of Chronic Disease Management, the responsibilities include: providing inpatient and outpatient education, clinical and self-management support for patients and families living with cardiovascular disease; working in collaboration with the inter-professional team in acute care and in an outpatient setting; individual telephone follow-up post discharge; staff education; collaboration with other SHA departments. Requirements

  • 1 year experience in adult ed/group facilitation with cardio disease mgmt
  • 1 year experience in program/resource development
  • 3 years clinical experience with patients/families living with cardio disease
  • Clinical and education experience with cardiac rehabilitation patients/families
  • Experience in teaching adults, facilitation skills, and leading group activities
  • Bachelor of Science in Nursing (BScN)
  • Valid Class 5 driver's license
  • Basic Life Support Certificate (BLS)
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)

Nice To Haves

  • Master of Nursing, Adult or Continuing Education is an asset
  • Experience coordinating, and facilitating adult education is an asset
