About The Position

The Patient Flow Coordinator is responsible for providing a single point of contact for clients discharging, transferring, admitting, and discharging within the SHA. This role involves coordinating the utilization of the electronic assessment system RAI-MDS for Long Term Care eligibility and placement, as well as alternate level of care services placement in the region. The coordinator will maintain the electronic bed utilization system for IRH and the SHA, and for the region. Additionally, the role requires implementing an effective program for continuous quality improvement and management practices to ensure efficient system utilization in consultation with Managers and Directors of Rural Health Services. Maintaining data integrity and submitting data to the region as required are also key responsibilities.

Requirements

  • A minimum of five (5) years current experience in long term care or home care
  • Demonstrated experience in MDS systems and interpreting MDS outcomes.
  • Demonstrated experience with staff development and training.
  • Bachelor of Science in Nursing (BScN)
  • Basic Life Support Certificate (BLS)
  • Valid Class 5 driver's license
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)

  • Bachelor of Psychiatric Nursing is acceptable
  • Psychiatric Nursing diploma is acceptable
  • Nursing diploma is acceptable
  • Demonstrated ability to manage resources and projects is considered an asset

Responsibilities

  • Providing single point of contact for clients discharging within the SHA.
  • Providing single point of contact for clients transfers, admissions and discharges with SHA.
  • Coordinating the utilization of the electronic assessment system RAI-MDS (MDS) as the Screening tool for Long Term Care eligibility and placement and alternate level of care services placement in the region.
  • Maintaining the electronic bed utilization system for IRH and the SHA.
  • Maintaining the electronic bed utilization system for the region.
  • Providing an effective program and continuous quality improvement and management practice to ensure efficient utilization of the system in consultation with Managers and Directors of Rural Health Services.
  • Maintaining data integrity and data submission to the region as required.
