About The Position

The System Flow Coordinator is responsible for providing a single point of contact for clients within the Saskatchewan Health Authority (SHA). This role facilitates physician consults and patient transfers from facility to facility to ensure appropriate patient and system flow across the SHA. The coordinator collaborates with other stakeholders to complete repatriation of patients back to their appropriate home hospital. This position is responsible for coordinating the utilization of the electronic assessment system RAI-MDS as the screening tool for Continuing Care eligibility and placement, and alternate level of care services placement within the SHA. The role also involves managing the placement of Continuing Care clients into appropriate vacancies and providing an effective program and continuous quality improvement and management practice to ensure efficient utilization of the system in consultation with Directors of assigned Integrated Service Areas. Maintaining and reviewing the Acute Care Dashboard for Bed management purposes is also a key responsibility.

Requirements

  • Five (5) years experience in Acute Care
  • 5 years demonstrated experience in MDS systems
  • A minimum of (5) years in Continuing Care of Home Care
  • Demonstrated experience in MDS systems and interpreting MDS outcomes.
  • Bachelor of Science in Nursing (BScN)
  • Valid Class 5 driver's license
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
  • Registered/eligible to be registered with RPNAS (Reg Psych Nurses Assoc of Sask)

Nice To Haves

  • Combination of experience in acute/continuing/home care preferred
  • Previous Coordinator experience is an asset
  • Demonstrated ability to manage resources and projects is considered an asset
