About The Position

The Neuromuscular Program Coordinator at UT Erlanger Neurology will carry out responsibilities which ensure high quality, well-coordinated patient care. The Coordinator is responsible for planning, coordinating, and evaluating services provided in the following areas; ALS Clinic, EDX Lab schedule, Neuromuscular physician schedule. The Coordinator is responsible for customer service initiatives, insurance pre-certifications, maintenance of EPIC schedule, quality metrics, coordination of care, including facilitation of diagnostic and therapeutic modalities. The Coordinator will work closely with the Regional Manager, Clinic Medical Director, and Neuromuscular Program Director to ensure that supplies, documentation, billing, scheduling, treatment modalities, referrals, and other follow up activities related to the care of patients presenting to this program. The Coordinator is responsible for developing professional and community relationships that will enable the provision of care and support to those living with Neuromuscular Disease, such as ALS and their families. This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices� patient population. Participates in the practice's quality improvement process and performance evaluation.

Requirements

  • Graduate from an accredited School of Nursing.
  • Two (2) years of hospital or physician practice experience.
  • Current Licensure to practice practical nursing in the State of Tennessee.
  • BLS completed prior to going to the unit/department and maintained going forward
  • Highly developed verbal and written communication skills
  • Ability to work effectively with administrative, clinical and support staff

  • Experience with billing, insurance pre-certifications, clinic scheduling and other aspects related to patient care coordination

Responsibilities

  • Administer vaccinations and monitor for adverse effects
  • Perform phlebotomy procedures and handle outside referral labs
  • Understand drug pharmacology in order to respond to patient medication concerns
  • Administer and coordinate care of medical equipment including feeding tubes, DME, and respiratory equipment
  • Order of labs, radiological studies, and referrals as dictated by ordering providers
  • Makes phone calls to all patients in ALS Clinic who have an appointment 48 hours in advance in effort to reduce/eliminate cancelations and/or no-shows
  • EPIC scheduling for patients presenting to the clinic who cover all aspects of the Neuromuscular Program
  • Collaborate with Clinic Practice Manager to order necessary supplies and equipment for the program
  • Collaborate with the Administrative Team in clinical program goal setting, progress evaluations, and ongoing problem solving
  • Maintain relationships with various sources of Program funding. Collaborate with the Neuroscience Administrator to appropriately allocate funds available.
  • Participate in conferences, staff meetings, training courses related to the Program
  • Serve as a resource to staff related to the Program
  • Oversee follow up documentation and evaluations to ensure patients have what is needed once the clinic visit is complete and based on physician order
  • Assist with billing for the Program
  • Assist with insurance pre-certifications for the Program
  • Maintain excellent patient service relationships and represent Erlanger in a professionally in all work related interactions with vendors, community members and healthcare colleagues
  • Participates in ongoing training and assistance with medical record maintenance
  • Assists all patients presenting to the program through the system by acting as a patient advocate/navigator
  • Participates in team meetings/ patient conferences
  • Participates in quality improvement initiatives for the Program to include but not limited to data collection, health outcomes reporting, clinical audits, and programmatic evaluation
  • Coordinates continuity of referrals to/from providers
  • Assists in the evaluation of clinical care, utilization of resources, and development of new clinical tools, forms, policies and procedures
  • Maintains confidentiality with all aspects of information in accordance with Hospital, physician practice, State and Federal regulations
  • Communicates with community health organizations and personnel in effort to provide outreach and awareness of the Program
  • Documents and tracks clinic volume
  • Responds to telephone and email inquiries related to the Program
  • Provides navigation assistance with regard to transitioning or applying for insurance and/or benefits
  • Develops and maintains active, accurate referral resource files
