About The Position

Under the direction of the Division Chair, the Network Diabetes Program Manager plans and directs the operational, financial, and personnel activities of the Network Diabetes Program. The role includes patient assessment, patient and clinical staff training, development of educational materials, and assistance with department budget preparation. The manager supervises certified diabetes educators, including nurses and dietitians, and coordinates county-wide program activities. The position applies American Diabetes Association standards of care in diabetes care, self-management, and food and nutrition. It includes travel to Cook County Health work sites throughout Cook County and work in a healthcare environment subject to hospital and department safety requirements, including personal protective equipment, hand hygiene, and applicable engineering and work-practice controls.

Requirements

Bachelor’s degree from an accredited college or university in Food and Nutrition, Public Health, or a related field; proof required at interview.
Two years of management or organizational experience in a healthcare setting.
Certified Diabetes Educator (CDE); proof required at interview.
Registered Dietitian (LDN) in the State of Illinois; proof required at interview.
Registered Dietitian by the Commission on Dietetic Registration; proof required at interview.
Ability to travel to CCH work sites.
Knowledge of diabetes care, diabetes self-management, food, and nutrition.
Ability to teach and conduct in-service trainings.
Ability to assess the impact of diet on diabetic patients and prepare education plans for people with diabetes.
Skill in applying current ADA standards in patient management.
Skill in delivering information to patients and clinical staff.
Ability to plan and develop nutrition education materials for patients.
Adherence to hospital and department safety requirements in the healthcare environment.

Nice To Haves

Master’s degree in Food and Nutrition, Public Health, or Business from an accredited college or university.

Responsibilities

Supervise certified diabetes educators, including nurses and dietitians.
Clinically assess patients with diabetes, including the impact of diet, and identify care plans that account for patients’ age-specific needs.
Assist with development of the fiscal budget.
Coordinate planning of county-wide Network Diabetes Program activities.
Plan and develop nutrition education materials for patients.
Train and supervise nurses, pharmacists, and other health promoters regarding nutrition management of patients.
Provide in-service training and orientation, and participate in activities associated with ADA recognition programs.
Adhere to ADA standards of care in diabetes care, diabetes self-management, and food and nutrition.
Travel to CCH work sites throughout Cook County as required.
Participate in American Diabetes Association programs.
Perform other duties as assigned.
Carry out applicable general administrative management responsibilities, including staff management and discipline, applying CCH policies and procedures, and working with Labor Relations on collective bargaining matters.
