About The Position

The Patient Navigator guides patients through the complexities of the healthcare system, collaborating with physicians, staff, and other healthcare professionals to coordinate care across the continuum for assigned patients. The role supports safety, best practices, and high-quality standards of care by connecting patients with available services, navigating insurance authorizations, coordinating care between providers, and helping ensure patients receive appropriate care. The Navigator collects and manages relevant clinical information before consultations, surgeries, or procedures; assists with authorizations and appointment scheduling as appropriate; and coordinates care plans, referrals, and follow-up. The role helps patients overcome barriers to treatment and supports them through communication by telehealth, in person, text, telephone, or email. It also provides educational resources as directed by the clinical team and serves as a resource for insurance and administrative aspects of peri-operative and peri-procedural care. The position works closely with physicians and multidisciplinary care management teams, facilitates and documents care coordination, extracts information from medical records, and communicates relevant details to the appropriate team, providers, and supervisors. Success is measured through process and outcome performance measures for the patient population. The work requires critical thinking, effective communication, decisive judgment, minimal supervision, and the ability to act appropriately in a stressful environment.
