About The Position

Access Healthcare Associates, part of Brigade Health and its Affiliates, is seeking a compassionate and driven Nurse Practitioner or Physician Assistant for its Mobile Primary Care Team. The role provides medical care to older adults in their homes and senior living communities, with a focus on clinical excellence, personal connection, and relationship-centered care. The organization serves communities across multiple states and combines clinician autonomy with teamwork and support from multidisciplinary experts in palliative, psychiatric, and neurological care. The position offers structured onboarding, coaching, and playbooks, and opportunities to develop expertise in dementia-informed care through the organization’s geri-psych and cognitive health initiatives. Compensation is a guaranteed base salary plus productivity, dependent on the number of days worked. The posted pay range is $140,000–$200,000.

Requirements

Active, unencumbered Nurse Practitioner or Physician Assistant license in the state where the clinician will practice.
Minimum of 1–2 years of clinical experience.
Compassionate, patient-centered approach and strong clinical judgment.
Valid driver’s license and reliable transportation.
