About The Position

Bluestone Physician Services is hiring a Nurse Practitioner or Physician Assistant to provide team-based primary care to patients in Assisted Living and Memory Care communities in and around Shoreview, Minnesota. The role is part of a multidisciplinary care model focused on preventative, proactive, individualized care for patients with complex chronic conditions and disabilities. Providers collaborate with patients, families, healthcare providers, and community partners to manage chronic conditions, address social determinants of health, coordinate transitions from inpatient settings, and provide behavioral health support. The position involves on-site care, managing a panel of approximately 200 patients, and seeing approximately 12–16 patients per day, with an average of 14. Full-time providers care for patients Monday through Thursday; Fridays may be used as a clinical or administrative day. Providers can design and manage their schedules and receive support from Clinical Assistants, LPNs, RNs, and Social Workers, with access to laboratory testing and x-rays as needed. Local travel to patient communities is required. Compensation is a base salary plus quality/productivity compensation, with a stated range of $125,000–$170,000. Requirements

Graduate of an accredited Nurse Practitioner or Physician Assistant education program.
Board certified or board prepared through AANP, ANCC, or NCCPA.
Licensed, or able to obtain licensure, through the applicable Board of Nursing or Board of Medical Practice.
Ability to obtain and/or maintain DEA certification for the state of employment.
Effective communication skills, including strong verbal skills.
Excellent interpersonal and customer service skills.
Valid and unrestricted driver's license and minimum state-required liability auto insurance.
Ability to travel locally.
