The Mission Case Manager implements strategies to achieve the goals for the organization and Mission Services. This role involves engaging service seekers in a collaborative planning process, conducting assessments, developing service plans, providing coaching and navigation, coordinating services with internal and external partners, maintaining regular contact with service seekers, supporting access to services, documenting services accurately, and participating in team meetings and continuous improvement activities. The role also includes project and change management, problem-solving, technical skill application, and community engagement.
