MERIT Centre Director La Crosse, WI $26.00 - $30.55 Hourly This position requires a valid, non probationary drivers license. Requirements

Require high school diploma or equivalent
Experience in supervisory and leadership position
Basic computer skills
Must possess valid driver’s license and be insurable

Nice To Haves

Prefer two or four-year degree in healthcare related field, or at least 2 years experience working in healthcare related field in which served population is similar to participant group at the MERIT Centres.

Responsibilities

Provide leadership and oversight to assure an uplifting and safe environment for participants through innovative programming designed for enjoyable participation.
Responsible for day-to-day operations and providing individualized, high quality care, services, and opportunities for participants in order to maintain or achieve their highest level of independence in accordance with the written service plan.
Provide on-going case management services to participants, ensuring the completion of pre-enrollment screening, assessment/reassessment and the individual service plan in conjunction with the participant, their family, associates, and other stakeholders.
Maintain compliance with appropriate local, state, federal, regulatory, and accrediting body standards.
Maintain all necessary paperwork for participants as required under the DHS 105.14 state code.
