About The Position

The Mental Health Caseworker/QMHP on the ACT Team assists individuals in accessing medical, social, educational, and other needed resources. The role provides rehabilitative and skills training, case management, care coordination, treatment planning, crisis services, and documentation of client progress. It also provides supported employment and telemedicine services.

Requirements

• Valid Texas driver's license and an acceptable driving record.
• Personal liability insurance as required by the state of Texas.
• Working computer skills, including Microsoft Office.

Responsibilities

• Assist individuals with access to medical, social, educational, and other needed resources.
• Provide rehabilitative services and skills training.
• Provide case management and care coordination.
• Develop treatment plans.
• Provide crisis services.
• Document client progress.
• Provide supported employment services.
• Provide telemedicine services.
