About The Position

The Mental Health Caseworker/QMHP works on an ACT Team, helping individuals access medical, social, educational, and other needed resources. The role includes rehabilitative and skills training, case management, care coordination, treatment planning, crisis services, and documenting client progress. The position also provides supported employment and telemedicine services.

Requirements

• Valid Texas driver's license.
• Acceptable driving record.
• Personal liability insurance as required by the State of Texas.
• Working computer skills, including Microsoft Office.

Responsibilities

• Assist individuals in gaining access to medical, social, educational, and other needed resources.
• Provide rehabilitative services and skills training.
• Provide case management and care coordination.
• Develop and support treatment planning.
• Provide crisis services.
• Document client progress.
• Provide supported employment services.
• Provide telemedicine services.
