Medication Access Coordinator

Jordan Valley Health•Springfield, MO

About The Position

The Medication Access Coordinator is a clinical population health team member embedded in a primary care clinic. The coordinator works directly with patients during scheduled healthcare visits to support medication adherence, pharmacy access, and continuity of medication therapy.
