About The Position

We offer a full range of healthcare services that include Medical, Behavioral Health, Dental, and Substance Abuse. This position works to support patient flow, support for clinical Providers, EMR documentation, and ensure a complete medical record for each patient.

Requirements

  • Bachelor's Degree (BA) from four-year college or university, or one to two years of related experience and/or training, or equivalent combination of education and experience.
  • Minimally 1-2 years experience working in clinical care environment, preferably ambulatory or community health center setting.
  • Computer skills required: Internet Software; Spreadsheet Software (Excel); Word Processing Software (Word); Electronic Mail Software (Outlook); and EMR.
  • Acute attention to detail

Nice To Haves

  • Bi-lingual skills are a plus but not required.

Responsibilities

  • Facilitate preparation for patient pre-operative visits, including contacting patient to gather information, ensuring external records are received, and faxing note, EKG and other relevant labs.
  • Support clinical visits through the EMR note preparation for well exams, emergency room follow-up, prenatal visits, and patients establishing care.
  • Conduct follow-up with patients recently discharged from the hospital or emergency room to ensure patient receives follow-up care post-discharge.
  • Coordinate DME orders, follow-up, and documentation
  • Place orders into the EMR at the direction of the clinical provider
  • Review the patient schedule and ensure external documentation has been received including discharge summaries, newborn screening, specialty consultations, and advanced imaging.
  • Support administrative functions directed by the clinical provider and medical team including faxing paperwork, writing work/school notes, patient form completion, navigating community resources, and supporting completion of care gaps.
  • Conducts patient follow-up for gaps in care to schedule appointments or help patient navigate completion of the gap. Coordination may include phone contact, in-clinic contact, and use of the text message system. Examples of gaps in care include immunizations due, cancer screening follow-up, lab testing due, chronic disease appointment scheduling, and collection of patient clinical information.
  • Support medical team with miscellaneous administrative tasks.
  • Support provider peer review process through data entry, EMR review, and result aggregation.
  • Performs other related duties as assigned by management.
