Requirements

  • Medical Social Worker with a current license in the state of employment
  • Minimum one-year experience as a MSW in an acute care setting
  • Master’s degree of Social Work required
  • Valid driver's license and auto insurance in your name as a driver
  • Capable of all physical demands

Responsibilities

  • Provide patients with access to community resources
  • Ensure that the patients’ medical, emotional and safety needs are met
  • Provide comprehensive skilled services as ordered by the attending physician
  • Educate patients and their family members and ensures the safety of the patient
  • Participate in the development of the Plan of Care
  • Conduct assessments of the patients’ needs and identifies any barriers
  • Observe, record, and report patient reactions or changes to appropriate agency staff
  • Connect patients and family to community resources
  • Assists with patient financial aid applications and long-range planning
  • Counsel the patient and family related to coping, changes and grief
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines
  • Understand and follow agency policies, procedures, rules, and regulations
  • Communicate changes in schedule/availability to schedulers or supervisors
  • Attend in-service training and mandatory agency meetings
