Requirements

Current Medical Social Worker license in the state of employment (limited licenses not accepted)
Minimum 1 year of MSW experience in an acute care setting (home care experience preferred)
Master’s degree in Social Work
Valid driver’s license and auto insurance
Ability to meet physical job demands

Responsibilities

Participate in developing patient care plans
Conduct patient assessments and identify barriers
Observe, document, and report patient changes to appropriate staff
Connect patients and families to community resources
Assist with financial aid applications and long-term planning
Counsel patients and families on coping, life changes, and grief
Complete clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
Follow all agency policies, procedures, and regulations
Communicate schedule or availability changes to supervisors
Attend required trainings and agency meetings
