About The Position

Fourth Street Clinic is hiring a full-time, onsite Medical Case Manager for Magnolia Apartments in Salt Lake City. The role is part of the Health and Housing Transition (HHoT) Team and supports individuals and families with a history of chronic homelessness who live in a permanent supportive housing program. The Medical Case Manager helps tenants navigate medical, social, educational, and other supportive services to promote their well-being.

The position works collaboratively with tenants and community partners, including The Road Home, to coordinate referrals and follow-ups for healthcare services, insurance eligibility, transportation, and needs related to social determinants of health. Fourth Street Clinic provides integrated healthcare to Utahns experiencing homelessness and describes its team as collaborative and committed to health equity and human dignity.

Requirements

• High school diploma or equivalent.
• Social service or community work experience.
• Obtain a Utah case management certificate within 90 days of hire.
• Must be able to pass a background check, including fingerprints.
• Strong communication skills.

Nice To Haves

• Bachelor's degree in social work, social services, or a similar field.
• Social Service Worker (SSW) Utah license.
• Experience in medical case management or care coordination.
• Experience working with homeless or vulnerable populations.
• Ability to communicate in Spanish.
