About The Position

Grays Harbor Health & Rehabilitation Center is seeking an MDS Coordinator (LPN) for its on-site facility in Aberdeen, Washington. The role combines clinical expertise and leadership to support quality outcomes, accurate assessments, regulatory compliance, and resident care. The MDS Coordinator works with interdisciplinary teams and serves as a clinical and operational partner within the facility. Pay is $44.00–$48.00 per hour, based on experience, qualifications, and skills. A $10,000 sign-on bonus is offered. Previous MDS experience in skilled nursing is required.

Requirements

• Current Washington LPN license.
• Previous MDS experience in skilled nursing is required.
• Strong knowledge of the RAI process and case management.
• Strong attention to detail and organizational abilities.
• Ability to work collaboratively with interdisciplinary teams.
• Ability to meet all employment screening requirements.

• Previous MDS Coordinator experience preferred.
