MDS Coordinator, RN or LPN, $5,000 Sign-On
About The Position
Journey is looking for an experienced MDS Coordinator to join their team at The Crossroads of Flowery Branch. This is a great opportunity for an RN or LPN with MDS, RAI, care planning, and long-term care experience who’s looking for a stable full-time role with a supportive team. The MDS Coordinator participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs of the residents and projected outcomes as required by Federal and State regulations.
Requirements
- Must hold a current nursing license in the state of employment.
- Working knowledge of the MDS 3.0 (current version of minimum data set).
- MDS experience
- RAI experience
- Care planning experience
- Long-term care experience
Responsibilities
- Conduct and coordinate MDS assessments, develop individualized care plans, and ensure timely updates for any significant changes in residents' conditions.
- Ensure accurate completion and submission of MDS assessments in accordance with Medicare/Medicaid guidelines and state/federal regulations.
- Facilitate care plan meetings, maintain effective communication across departments, and support continuous improvements in resident care outcomes.
