About The Position

Volare Health is seeking an experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join their team as an MDS Coordinator at South Hills Rehabilitation Center in Eugene, OR. This role is crucial in partnering with the interdisciplinary care team to ensure accurate resident assessments, regulatory compliance, and optimal reimbursement. The MDS Coordinator is responsible for managing the MDS process, supporting high-quality resident care through timely and accurate clinical documentation, and contributing to a welcoming environment for residents, families, and team members. Requirements

  • Current, active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Oregon.
  • Knowledge of the Resident Assessment Instrument (RAI) process and case management.
  • Strong communication, organization, and interpersonal skills.
  • Ability to work collaboratively within an interdisciplinary team.

Nice To Haves

  • Previous MDS experience in a skilled nursing or long-term care setting preferred.

Responsibilities

  • Participate in the pre-admission process to obtain essential clinical information needed for accurate MDS optimization.
  • Collaborate with the interdisciplinary team to review resident care and establish the most appropriate Assessment Reference Date (ARD) for Prospective Payment System (PPS) MDS assessments.
  • Ensure resident assessments are completed accurately and on time in accordance with state and federal regulations.
  • Complete precise MDS coding using information gathered through resident interviews, family discussions, staff collaboration, direct observation, and medical record review.
  • Maintain and manage MDS assessment schedules to ensure regulatory compliance.
  • Perform additional duties as assigned to support resident care and department operations.
