About The Position

This professional Registered/Licensed Nurse is responsible for coordinating nursing care for patients in ECHRS unit by providing a holistic approach to patient needs. Responsible for completing MDS assessments on a caseload of patient/resident ensuring all regulatory requirements and deadlines are met. Must be able to complete concise, accurate and detailed documentation and chart reviews. Must be willing to complete continuing education related to MDS independently as well as scheduled trainings, in-services etc. Attends care conference meetings with patients/residents and responsible parties. Attends all required IDT meetings including but not limited to: Medicare Meeting, Behavior Management, Morning Meeting, PPS, GG, etc. Must be flexible with work schedule as there are potential mandates as well as one scheduled weekly late night and every 5th weekend on call expected. Responsible for learning rolls of entire CCC team (MDS, Admissions and Discharge Planning) and covering for others on the team as needed. Other duties as needed.

Requirements

• Ability to cooperate with others and use tact and patience in dealing with residents, their families and visitors.
• Detail oriented and able to multi task.
• Ability to be flexible and be capable of adapting to frequent changes including work hours, daily routine, etc.
• Experience as a general staff nurse, in which administrative and supervisory ability has been demonstrated.
• Graduation from an accredited school of nursing and current registration/licensing with the State Board of Nursing Examiners.

Nice To Haves

• MDS and case management experience is preferred but not required.
