MDS Coordinator RN - $5,000 sign-on
About The Position
Avalon Care Center at Northpointe is hiring a full-time MDS Coordinator RN in Spokane, Washington. The role leads the MDS and resident assessment process, supports assessment accuracy and timely documentation, coordinates care planning with the interdisciplinary team, and helps ensure compliance with Medicare and Managed Care requirements. The MDS Coordinator contributes to quality outcomes, accurate reimbursement, and the facility’s clinical and financial success, while serving as a resource for MDS, care planning, and compliance. The position offers a consistent schedule with no nursing call rotation and includes a $5,000 sign-on bonus. Pay is $60.00–$65.00 per hour and is based on experience, qualifications, and skills.
Requirements
- Current Washington RN license or eligibility under the Nurse Licensure Compact (NLC).
- Previous MDS and skilled nursing facility experience.
- Strong knowledge of the RAI process and case management.
- Strong communication and organizational skills.
- Ability to pass required employment screenings.
Responsibilities
- Lead the MDS and resident assessment process.

- Coordinate care planning with the interdisciplinary team.
