Requirements

  • Bachelor’s degree in Nursing
  • one (1) to three (3) years of long-term care MDS coordination and care planning experience
  • Valid Registered Nurse license required
  • Incumbent to have or obtain Resident Assessment Coordinator certification within one year of hire.
  • Incumbent should be able to apply common sense understanding to carry out instructions furnished in written and oral form
  • Possess high-quality verbal and written communication ability
  • Strong customer service and interpersonal skills.
  • Proficient in the use of various computer applications
  • Knowledgeable on all state and federal health regulations.

Responsibilities

  • Complies with all CMS guidelines for the completion of the Minimum Data Set
  • Establishes the assessment reference data for all Minimum Data Sets under the Medicare Patient Driven Payment Model to capture the highest resident reimbursement.
  • Coordinates the need for billable MDS with the third party billing in the Finance department.
  • Meets with families and the Director of Nursing, resident, social workers, nursing staff, dietary, activities, etc. to collect medical, hospital, social, dietary and family records of each newly admitted resident.
  • Inputs MDS, reentry, and discharge tracking data in to the computer in an accurate and efficient manner.
  • Gathers all initial utilization data and reports results at the Utilization Data review meeting.
  • Complies with Medicare PDPM (Patient Driven Payment Model) in conjunction with the interdisciplinary team considering status changes, quarterly reviews and annual reviews.
  • Monitors changes in the Minimum Data Sets and inputs changes of data in to the computer.
  • Reports changes to Director of Nursing as appropriate.
  • Develops, implements, and maintains an ongoing quality management program for the resident assessment/care plan.
  • Ensures that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties, including civil money penalties, for false certification.
  • Completes all MDS reporting by the set date.
  • Participates in the daily clinical meeting to address resident issues for potential variances in the MDS assessment/care plan.
  • Completes monthly review of the Casper report for opportunity of improvement in Quality Measures.
  • Works with the Care Plan team to develop individualized care plans for all residents of the community:
  • Acts as a leader and manager of assigned positions; ensuring productivity, efficiency and accuracy in work performed:
  • Maintains professional presence when representing Ingleside exhibits degree of professionalism consistent with standards in performance, behavior, and appearance.
  • Demonstrates professional development/skills competency in all activities.
  • Complies with Ingleside established safety rules and practices.
