About The Position

The Massachusetts Veterans Home (MVH) at Holyoke is seeking an experienced, collaborative and dynamic individual to serve as MDS Clinical Nurse Supervisor. This role will serve as a key member of the nursing team in managing the overall MDS process. The ideal candidate will possess a strong background in nursing with experience in long-term care. This role requires excellent clinical skills, attention to detail, and the ability to work independently while ensuring adherence to healthcare regulations and standards. Applicants must have RAC-CT special certification and MDS experience to be considered.

Requirements

  • RAC-CT with at least 1 year of experience in MDS
  • Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing
  • Current CPR certification
  • Ability to keep current on changes in MDS and regulations affecting nursing practice.
  • Experience with MDS compliance including CMS standards.
  • Ability to teach and mentor others.
  • Ability to manage conflict and collaborate with others across the multidisciplinary care team.
  • Leadership skills including effective verbal and nonverbal professional communication, critical thinking, conflict management and effective decision making.
  • Ability to explain the laws, rules, regulations, policies, procedures, specifications, standards and guidelines governing agency operations and assigned unit activities.
  • Ability to maintain accurate records.
  • Ability to read and interpret documents such as physicians' orders and treatment plans.
  • Comprehensive understanding of VA and MA regulations regarding long term care
  • At least two years of full-time, or equivalent part-time, experience as a registered nurse in a recognized hospital, clinic or medical facility
  • Of which at least one year must have been in a supervisory, administrative or managerial capacity or any equivalent combination of the required experience and the substitutions below.
  • A Graduate degree with a major in Nursing may be substituted for a maximum of one year of the required (A) experience. No substitutions will be allowed for the required (B) experience.

Responsibilities

  • Collaborating and communicating with appropriate departments to ensure that all sections of the MDS and corresponding assessments are completed by reference date.
  • Ensuring the Facility Matrix (CMS 802) is up to date and accurate.
  • Educating staff on the MDS process, conducting audits, monitoring compliance, collecting data and submitting reports.
  • Coordinating the MDS schedule and collaborating with nursing and social work regarding the planning and execution to the nursing team.
  • Functioning as an MDS resource for all departments involved with the resident assessment instruments process.
  • Training others on PPS, appropriate combining of assessments and PDPM.
  • Educating and training new RNs on the MDS process.
  • Educating ancillary departments and providers of documentation for MDS accuracy.
  • Completing audits on the MDS process, related assessments and quarterly summaries by all shifts.
  • Transmitting all data in the format specified by the VA and CMS within regulations.
  • Assisting with admissions/readmissions of the veterans when assigned.
  • Assisting nursing and social work with the scheduling and planning of IDC meetings to include notification of participants.
  • Overseeing compliance regarding the completion and documentation of quarterly summaries and significant changes and all associated assessment tools, but limited to the Norton Scale, Fall Risk Assessments, Smoking and Wander Risk Assessments.
  • Coordinating, developing and providing in-service education programs.
  • Maintaining Quality Assurance/Performance Improvement data and reporting out at least quarterly.
  • Keeping the IDT abreast of changes in RAI manual affecting their departments and documentation.
  • Providing reports and recommendations to leadership.
  • Completing employee performance appraisals (EPRS) on direct reporting staff.
  • Providing direct supervision of reporting staff.
  • Serving as a member of appropriate MVH clinical committees as needed.
  • Continuing professional growth by attending workshops, seminars, work related courses, conferences and staff meetings to maintain professional proficiencies and/or licensure.
  • Performing all other related duties as assigned.
