About The Position

The MDS Nurse RN is responsible for the coordination of the Resident Assessment Instrument (RAI), including the electronic transmission of RAI data to the state in accordance with State and Federal regulations and facility policies. This role also facilitates communication between the Interdisciplinary Team and provides ongoing education to those preparing the MDS.

Responsibilities

  • Coordinate the Resident Assessment Instrument (RAI) process.
  • Electronically transmit RAI data to the state.
  • Facilitate communication between the Interdisciplinary Team.
  • Provide ongoing education to staff involved in MDS preparation.
  • Schedule completion of RAI following State and Federal guidelines.
  • Notify Interdisciplinary Team members of assessment schedules.
  • Assist in the completion of RAI as needed.
  • Assist with data input into the RAI computer system.
  • Supervise the completion of the RAI by the Interdisciplinary Team.
  • Modify assessment schedules to ensure appropriate RUG score for each resident.
  • Screen each Minimum Data Set (MDS) for accuracy before finalizing, locking, and signing.
  • Transmit RAI data to the state weekly or as needed.
  • Track Validation Reports, Monthly and Quarterly Reports from the state.
  • Actively participate in daily conferences with the Interdisciplinary Team to evaluate resident status and needs relative to Medicare skilled coverage and potential RUG category changes.
  • Assist Interdisciplinary staff with Medicare review via chart audit.
  • Ensure Change of Condition RAIs are initiated when appropriate.
  • Review chart documentation to ensure it reflects the need for resident care and services.
  • Ensure Medicare coverage is discontinued when a resident no longer meets criteria.
  • Ensure RAI/MDS software is functioning properly and is in contact with the software vendor for the most current version.
  • Clarify Medicare Certification/Re-Certification forms.
  • Assist with the completion and maintenance of the Resident Roster and Census and Condition Reports.
  • Work closely with the state RAI/MDS automation coordinator.
  • Coordinate with the billing department on assessment schedules and billing assessment codes.
  • Utilize the Internet for research on changes in the RAI and Medicare process.
  • Act as a liaison to the Interdisciplinary Team.
  • Perform other duties incidental to the position and as delegated by the Director of Nursing.
