The MDS Nurse is responsible for maintaining a complete and accurate MDS program, by ensuring completion of all facility, state and federal resident assessment forms (MDS) and assisting in the development and implementation of each resident's plan of care. The MDS Coordinator audits, and submits all required resident assessment forms (MDS) and necessary corrections of the MDS to the State of Ohio. Coordinates the RAI process; reviews care plans prior to care conferences; completes CAA assessments in a timely manner; updates ODH forms for survey; and continuously reviews residents' conditions for need to update MDS's Accountable for team productivity and the timely completion of delegated work assignments in accordance with established policies and standards. Participates and/or approves the evaluation of personnel. Assist in initiation, completion of MDS and other facility, state and federal required assessment forms. Coordinate residents' level of care and assessment schedules with other departments within the facility, therapy providers and consultants. Assist with the initiation and completion of residents' plan of care. Assist with staff education and development of facility forms and tools to ensure accurate assessment of residents' needs. Obtain, distribute and review Q.M.'s monthly from the State. Update PCC as need and assign passwords for staff. Monitor Nursing documentation ensure completeness and accuracy.
