About The Position

Stonebridge, an in-home personal care agency serving communities across Utah, is seeking a Marketer to manage referral sources and patient intake. The role focuses on maintaining and developing relationships with referral sources, managing incoming referrals, completing patient intakes, and coordinating timely care initiation. The Marketer also supports marketing initiatives and serves as a liaison among referral sources, patients, and internal care teams. This is a full-time community liaison role with an opportunity to grow into a Director of Marketing role as Stonebridge grows. Compensation is hourly, with mileage reimbursement for intakes and referral check-ins; details will be discussed during the interview.

Nice To Haves

• Previous experience in home health, personal care, or case management is preferred.
• Marketing or referral development experience is a plus but is not required.

Responsibilities

• Manage incoming referrals from hospitals, skilled nursing facilities, case managers, and other partners.
• Generate new referral sources and referrals from established referral sources.
• Perform patient intakes and execute service agreement contracts as needed.
• Coordinate care initiation promptly to reduce delays and improve the patient experience.
• Maintain accurate referral and intake trackers.
• Build and maintain relationships with referral sources through regular communication and check-ins.
• Identify growth opportunities and assist with marketing initiatives as needed.
• Serve as a liaison between referral sources, patients, and internal care teams.
