About The Position

The Manager of Care Management, under the guidance of the Director of Care Management, provides leadership, coaching, and mentoring to the Care Management department’s professional and administrative staff. The role supports and ensures that service-delivery standards of practice and organizational performance strategies are consistent with PHC’s Mission, Vision, and Values. The manager also provides Care Management services to hospital patients, as appropriate, to guide care facilitation, coordination, and transitions of care.

Requirements

Graduate from a recognized, accredited school of nursing, or hold a bachelor’s degree in social work, healthcare administration, or another related field.
Five years of experience. For nurses, this must be a combination of nursing and Care Management experience in a hospital setting and include leadership or management experience in an acute care setting.
For nurse candidates: current Registered Nurse (RN) licensure in Georgia or a compact state, required upon hire.
For social worker candidates: current unrestricted Licensed Master Social Worker (LMSW) licensure in Georgia, required upon hire.

Nice To Haves

Certified Case Manager.
Social Work Case Manager Certification (CSWCM).
