About The Position

Case Managers facilitate direct patient care and skilled nursing visits in a home health setting. Tasks include medication management, wound care, vital sign monitoring, and patient education in accordance with the established plan of care. The role is scheduled Monday through Wednesday, 9 a.m. to 3 p.m., with no weekends or holidays.

Requirements

• Hold a current license as a Licensed Practical Nurse or Registered Nurse from the Virginia Board of Nursing.
• Have experience working in a home care agency.
• Have direct patient care experience.
• Be dedicated and dependable.

Responsibilities

• Complete patient care duties.
• Follow the patient plan of care and reevaluate it as necessary.
• Work the scheduled shift to perform patient duties.
• Complete clinical nurse notes in their entirety.
• Complete skilled care services as applicable.
• Perform additional duties as assigned.
• Facilitate direct patient care and skilled nursing visits, including medication management, wound care, vital sign monitoring, and patient education.
