About The Position

The Staff LPN implements, directs, communicates, and documents the nursing process while providing a safe environment for patients, visitors, and associates. The LPN practices in accordance with established standards of care, the Nurse Practice Act, and Powers Health policies and procedures. This position serves Hartsfield Village Assisted Living, a residential setting for individuals who need some assistance with everyday tasks. The community has nursing presence 24 hours a day, seven days a week, to assist residents with activities of daily living.

Requirements

Graduate from an accredited school of practical nursing.
Currently licensed as an LPN by the State of Indiana Health Professions Bureau.
Obtain and maintain current BLS certification and other required certificates in specialty areas.

Responsibilities

Document a physical assessment each shift, including subjective and objective data.
Communicate with patients, families, and/or significant others to individualize each patient’s plan of care.
Verify that physician’s orders are transcribed properly and carried out promptly.
Report significant changes in a patient’s condition to the unit manager, charge nurse, shift supervisor, and/or physician.
Continuously evaluate patient responses to the plan of care, teaching plans, and rehabilitation activities throughout their stay and upon discharge.
