The Hospital at Home LPN provides direct care to assigned patients in an inpatient-level care model delivered in patients’ homes. The LPN works as a team member, assists other staff with patient care, and assumes responsibility for assigned patients under the direction of a co-assigned RN or MD. The role participates in care planning, implements appropriate nursing interventions, and supports patient and family education through data collection, interdisciplinary collaboration, documentation, and effective communication. This newly launched program provides acute hospital-level care in patients’ homes rather than in a traditional hospital unit. The LPN’s primary functions include assisting with admissions and discharges, remote patient monitoring, care coordination, and logistics management. The position requires recent acute care, ICU, or ED experience, a valid New York State driver’s license, and reliable transportation.
