LPN - Continuous Care - Day Shift
About The Position
Tidewell Hospice, part of Empath Health, is hiring an LPN to provide continuous bedside care to hospice patients across Manatee, Charlotte, DeSoto, and Sarasota counties. The position works under the direction of an RN case manager, providing clinical observations, prescribed care, and physical and emotional support as directed in the plan of care. The LPN reports significant changes in patient condition and family needs and collaborates with the RN to accept and process physician orders to maximize patient comfort. When continuous-care cases are pending overnight, LPNs also assist Triage and Care Centers. The schedule consists of flexible 12-hour shifts, and mileage is reimbursed.
Requirements
- Graduate of an approved nursing program.
- Current Florida LPN license.
- At least one year of experience in an acute care setting providing direct patient care.

- Previous experience as a hospice LPN.
Responsibilities
- Provide continuous bedside clinical observations and prescribed care under the direction of the RN case manager and as directed in the plan of care.
- Provide physical and emotional support to patients.
- Report significant changes in patient condition and family needs as needed.
- Collaborate with the RN to accept and process physician orders to maximize patient comfort.
- Assist Triage and Care Centers overnight when Continuous Care cases are pending.
